Quick Answer
Medicare Savings Programs (MSPs) are state-administered programs funded jointly by states and the federal government. They help people with limited income and resources pay Medicare premiums, deductibles, and copays. In 2026, the four MSP types cover different levels of costs: QMB pays the most (Part A & B premiums + cost-sharing), while QDWI covers only Part A premiums for a narrow group.
Coverage Comparison by Plan Type
| Plan Type | Coverage | Notes |
|---|---|---|
| QMB (Qualified Medicare Beneficiary) | Most Comprehensive | Pays Part A & B premiums + deductibles + copays |
| SLMB (Specified Low-Income Medicare Beneficiary) | Part B Premium Only | Pays Part B premium only |
| QI (Qualifying Individual) | Part B Premium Only | Pays Part B premium; limited slots, first-come basis |
| QDWI (Qualified Disabled & Working Individual) | Part A Premium Only | For disabled workers who lost premium-free Part A |
Understanding Your Coverage Options
QMB - Qualified Medicare Beneficiary
QMB is the most comprehensive Medicare Savings Program. It pays your Part A premium (if applicable), Part B premium ($202.90/month in 2026), Part A and Part B deductibles, and Part A and Part B copays and coinsurance. QMB enrollees are also automatically enrolled in Extra Help (Low Income Subsidy) for Part D.
Providers are prohibited by law from billing QMB enrollees for Medicare cost-sharing. If a provider bills you for copays or deductibles and you have QMB, you have the right to refuse payment and report the billing to your State Medical Assistance office.
What It Covers
- 2026 income limit: $1,255/month (individual), $1,704/month (couple)
- Resource limit: $9,660 (individual), $14,470 (couple) - excludes home, one car, burial funds
- Pays Part A & B premiums, deductibles, copays, and coinsurance
- Providers cannot legally bill QMB enrollees for Medicare cost-sharing
- Automatically qualifies you for Extra Help (Part D LIS)
What It Doesn't Cover
- Does not cover Part D drug costs (separate Extra Help program applies)
- Does not cover Medigap premiums
- Does not cover Medicare Advantage plan premiums beyond Part B
SLMB - Specified Low-Income Medicare Beneficiary
SLMB pays your Part B premium ($202.90/month in 2026) only. It does not pay deductibles, copays, or coinsurance. Like QMB, SLMB automatically qualifies you for Extra Help for Part D.
What It Covers
- 2026 income limit: $1,506/month (individual), $2,044/month (couple)
- Resource limit: $9,660 (individual), $14,470 (couple)
- Pays Part B premium only ($202.90/month in 2026)
- Does not cover deductibles, copays, or coinsurance
- Automatically qualifies you for Extra Help (Part D LIS)
What It Doesn't Cover
- Does not pay Part A premiums or cost-sharing
- Does not cover Part D costs (apply separately for Extra Help)
- Does not cover dental, vision, or hearing services
QI - Qualifying Individual
QI pays your Part B premium only, similar to SLMB, but has a slightly higher income limit. QI funding is limited - states receive a fixed annual allotment and enroll beneficiaries on a first-come, first-served basis. You must reapply each year, and prior QI enrollees get priority.
Unlike QMB and SLMB, QI does not automatically qualify you for Extra Help, but you can apply for Extra Help separately.
What It Covers
- 2026 income limit: $1,694/month (individual), $2,298/month (couple)
- Resource limit: $9,660 (individual), $14,470 (couple)
- Pays Part B premium only
- Limited slots - first-come, first-served; reapply annually
- Does NOT automatically qualify for Extra Help
What It Doesn't Cover
- Does not cover Part A premiums or cost-sharing
- Does not cover Part D costs - apply for Extra Help separately
- Slots are limited - not all eligible applicants may be enrolled
Apply Early for QI
QI funding is capped each year. Apply as early as possible - states stop accepting applications once the allotment is exhausted. Prior enrollees get priority.
QDWI - Qualified Disabled and Working Individual
QDWI is a narrow program for disabled individuals under 65 who lost premium-free Part A because they returned to work. It pays the Part A premium ($505/month in 2026 for those with fewer than 30 quarters of work). QDWI has stricter income and resource limits than the other MSPs.
What It Covers
- 2026 income limit: $4,615/month (individual), $6,189/month (couple) - but must not qualify for Medicaid
- Resource limit: $4,000 (individual), $6,000 (couple)
- Pays Part A premium only ($505/month in 2026 for under 30 quarters)
- For disabled workers under 65 who returned to work
- Does NOT pay Part B premium or cost-sharing
What It Doesn't Cover
- Does not cover Part B premiums or cost-sharing
- Does not cover Part D costs
- Only available to working disabled individuals - not standard retirees
How to Apply for a Medicare Savings Program
MSPs are administered by state Medicaid agencies, not Medicare directly. To apply, contact your state Medicaid office or call 1-800-MEDICARE (1-800-633-4227). You can also apply online in many states through Benefits.gov.
If you are already enrolled in Medicaid, you may automatically qualify for QMB. States are required to screen all Medicaid applicants for MSP eligibility.
What It Covers
- Apply through your state Medicaid office or Benefits.gov
- Call 1-800-MEDICARE for referral to your state office
- Medicaid enrollees may be auto-enrolled in QMB
- Benefits are retroactive to the month you apply in most states
- Reapply annually - benefits do not automatically renew in all states
What It Doesn't Cover
- Does not guarantee enrollment - income and resource limits apply
- Does not cover Medigap or Medicare Advantage plan premiums
- Does not provide retroactive coverage before application date

✦ Important Exceptions and Notes
Some States Have Higher Income Limits
Federal rules set minimum income limits, but states can choose to be more generous. Connecticut, Maine, and several other states have higher MSP income thresholds. Always check with your state Medicaid office for the most current limits.
QMB Billing Protections
Providers who accept Medicare are prohibited from billing QMB enrollees for Medicare cost-sharing. If you are billed, you can report the provider to your State Medical Assistance office or call 1-800-MEDICARE.
Resources That Don't Count
Your primary home, one vehicle, household goods, burial funds (up to $1,500), and life insurance with cash value under $1,500 are generally excluded from resource calculations.
MSP and Medigap
If you have QMB, you generally do not need a Medigap plan because QMB covers most cost-sharing. However, if you have SLMB or QI (which only pay Part B premium), a Medigap plan can still help with deductibles and copays.
