How Medicare and VA Benefits Work Together

Independent Programs

Medicare and VA benefits are completely separate programs that do not coordinate with each other. Unlike other insurance combinations where one pays first and the other pays second, Medicare and VA benefits each cover their own services independently. VA pays for care received at VA facilities; Medicare pays for care received at non-VA facilities.

This means you cannot use Medicare to pay for care at a VA facility, and you cannot use VA benefits to pay for care at a non-VA facility (with some exceptions like VA Community Care). Having both programs gives you access to a much wider range of providers and services.

  • VA: care at VA medical centers, clinics, and VA Community Care (authorized non-VA providers)
  • Medicare: care at any Medicare-participating provider (hospitals, doctors, specialists)

Do You Need Medicare if You Have VA Benefits?

Part A is usually free (premium-free if you have 40+ work quarters) and provides hospital coverage outside the VA. Part B ($202.90/month in 2026) adds outpatient coverage. If you delay Part B without qualifying coverage, you face a permanent 10% penalty per year - VA benefits do NOT qualify as creditable coverage for Part B delay purposes.

VA Benefits do not Qualify for Part B Delay

Unlike employer group coverage, VA benefits do NOT allow you to delay Medicare Part B without penalty. If you delay Part B past your Initial Enrollment Period, you will owe a permanent 10% penalty per year missed.

VA Drug Coverage and Medicare Part D

VA = Creditable Coverage

VA drug coverage IS considered creditable coverage for Medicare Part D purposes. This means you can delay enrolling in a Part D plan without penalty as long as you have VA drug coverage. If you later lose VA drug coverage, you have a 63-day window to enroll in Part D without penalty.

Many veterans choose to use VA drug coverage instead of Part D because VA drug prices are often lower, especially for generic medications. However, with the 2026 Part D $2,000 OOP cap and 10 negotiated drug prices, Part D may now be competitive with VA pricing for some medications.

Medigap and VA Benefits

Optional but Useful

Veterans who enroll in Medicare Part B can also purchase a Medigap (Medicare Supplement) plan to cover Part B cost-sharing (deductible, 20% coinsurance). This is particularly valuable for veterans who use non-VA providers frequently.

However, if you primarily use VA facilities for most of your care, the value of Medigap may be limited. Consider your usage patterns: if you use Medicare providers regularly, Medigap Plan G ($80–$200/month) can eliminate most out-of-pocket costs for Medicare services.

VA Community Care and Medicare

Non-VA Provider Access

VA Community Care allows veterans to receive care from non-VA providers when the VA cannot provide timely or geographically accessible care. When you receive VA Community Care, the VA pays - not Medicare. You cannot bill Medicare for VA Community Care services.

If you receive care from a non-VA provider that is NOT authorized by VA Community Care, Medicare (if you have it) would be the payer. This is why having Medicare is valuable - it covers non-VA care that is not authorized by VA Community Care.

  • Medicare: non-VA providers NOT authorized by VA - Medicare pays

Important Exceptions and Considerations

TRICARE for Life (TFL) vs. VA

Military retirees may have TRICARE For Life in addition to VA benefits. TFL wraps around Medicare (pays after Medicare), while VA is separate. Having all three provides maximum coverage.

VA Aid & Attendance

Veterans who need help with daily activities may qualify for VA Aid & Attendance pension benefits, which can help pay for home care or assisted living costs not covered by Medicare or VA health benefits.
