But choosing the right Medigap policy is not as simple as picking the plan with the lowest premium. You need to understand the standardized benefit structure, when to enroll to lock in the best price, how carriers price their premiums over time, and what to look for when comparing companies offering identical coverage.

Step 1: Understand That All Medigap Plans Are Standardized

Federal law requires that Medigap plans be standardized by letter. Every insurer selling Plan G must offer the exact same Plan G benefits, regardless of the company name or the premium charged. Plan G vs. Plan N: The Most Common Decision

Plan G and Plan N are the two most popular choices for new enrollees. Plan G offers the most comprehensive coverage available, with your only out-of-pocket cost being the $257 annual Part B deductible. Plan N has lower premiums but adds a $20 copay at office visits and a $50 copay at ER visits, and does not cover Part B excess charges. If you see doctors frequently, Plan G may offer better value. If you are generally healthy and want a lower monthly premium, Plan N can save money.

Step 3: Enroll at the Right Time

The single most important factor in choosing a Medigap policy is when you enroll. Your Medigap Open Enrollment Period (OEP) is a one-time, 6-month window that begins the month you are both age 65 or older and enrolled in Medicare Part B. During this window, you have guaranteed issue rights: insurers cannot deny you coverage, charge you more, or impose waiting periods based on pre-existing conditions.

Once your OEP closes, insurers in most states can use medical underwriting to evaluate your health history. Common conditions such as diabetes, heart disease, COPD, or a history of cancer can result in denial or significantly higher premiums. Premium differences for the same plan letter in the same ZIP code can vary by $50 to $150 per month or more between carriers. Over 10 years, that difference can amount to $6,000 to $18,000 in additional costs for identical coverage.

  • Premium: Get quotes from multiple carriers for the same plan letter in your ZIP code. Use a licensed Medicare agent who represents multiple carriers rather than a single company.

  • Pricing method: Ask each carrier which method they use. Two carriers offering Plan G at the same price today may have very different premiums in 10 years if one uses attained-age and the other uses community-rated pricing.

  • AM Best financial strength rating: Look for carriers rated A or higher by AM Best. This indicates financial stability and the ability to pay claims long-term.

  • Rate increase history: Ask how much the carrier has raised premiums in your state over the past 5 years. Some carriers are known for aggressive rate increases; others have more stable histories.

Step 6: Know Your State-Specific Rules

Most states follow the federal standardized plan structure, but a few have their own rules. Massachusetts, Minnesota, and Wisconsin have their own Medigap plan structures that differ from the federal standard. Additionally, several states have enacted birthday rules or anniversary rules that give existing Medigap policyholders a limited annual window to switch to a plan with equal or lesser benefits without underwriting.   2. Get quotes from at least 3 to 5 carriers for your chosen plan letter in your ZIP code. Use a licensed independent Medicare agent who can access multiple carriers simultaneously.

  3. Ask about the pricing method for each quote. If two quotes are similar today, the one using community-rated or issue-age pricing may be cheaper in 10 years than an attained-age plan.

  4. Check the carrier financial rating (AM Best A or higher) and ask about their rate increase history in your state.
